ADN Program Admission Criteria
Required Supporting Documentation
1. Academic degrees, relevant health care license or certificates held by an applicant, health care
work experience in direct patient care or health care volunteer experience in directpatient care Maximum TOTAL Points for Criterion 1 A, B, C & D = 15 points
Maximum points for Criterion 1A = 5 points
BA/BS Degree or higher
– OR –
Official transcript from regionally accredited U.S. colleges or universities with degree posted
– OR – If degree from outside the United States, must have been evaluated by a National
Association of Credential Evaluation Services (NACES) approved independent agency
(evaluation must be in a sealed envelope from the agency), demonstrating equivalency to a
BA/BS degree or higher from a U.S. regionally accredited college. The NACES evaluation will be
used ONLY to award points as part of ADN admission criteria – not for graduation purposes..
Official transcript from regionally accredited U.S. colleges or universities with degree posted

1B. Licensed health care and work experience as a LVN, Psychiatric Technician,
Radiologic Technician, Respiratory Therapist or Paramedic
Maximum points for Criterion 1B = 10 points
Currently licensed as a LVN, Psychiatric Technician, Radiologic Technician,
Respiratory Therapist, or Paramedic
Copy of current license with license number, issue date and expiration date.
LVN, Psychiatric Technician, Radiologic Technician, Respiratory Therapist or
Paramedic work experience in direct patient care with greater than 1000 hours
of experience
Letter from current/former employee verifying employment. The letter must be on organization
letterhead with an original signature and must include applicant’s name, start date and end date,
employee status (full-time/part-time), total number of hours worked, job title, department if
applicable, and examples of duties including direct patient care.
1C. Certified as a CNA, HHA, EMT and work experience as a CNA, HHA, or EMT
Maximum points for Criterion 1C = 5 points
Currently certified as a CNA, HHA, or EMT
Copy of current certificate including the certificate number, issue date and expiration date.
CNA, HHA, or EMT work experience in direct patient care with greater than
1000 hours of experience
Letter from current/former employee verifying employment. The letter must be on organization
letterhead with an original signature and must include applicant’s name, start date and end date,
employee status (full-time/part-time), total number of hours worked, job title, department if
applicable, and examples of duties including direct patient care.
1D. Certificates, licenses and health care work experience not covered under Criterion 1B
or Criterion 1C and/or volunteer experience in health care with direct human patient care
not covered in Criterion 1B. or 1C.
Maximum points for Criterion 1D = 2 points
Currently licensed or certificated health care employee not covered in Criterion
1B or Criterion 1C. (Certificate must reflect training > 70 hours)
Copy of current license or certificate.
Work experience in health care field with direct human patient care and greater
than 1000 hours of experience
Letter from employer(s) on organization letterhead verifying employment with an original
signature. Must include applicant’s name, start and end date, employee status (full-time/part-
time), total number of hours worked, job title, department if applicable, and examples of duties
including direct patient care;
Volunteer with direct human patient care with greater than 200 hours of
experience
Letter from organization(s) on organization letterhead verifying volunteer service with an
original signature. Must include applicant’s name, start and end date, total hours volunteered,
and examples of duties including direct human patient care. Providing or assisting human clients
with hands on health care needs, not including clerical or administrative duties.
